Custom Order Form – If We Don’t Have It, Let Us Create a Custom Medallion for Your Hero

We produce fully custom, raised-relief 3D medallions manufactured exclusively to your approved design. Each medallion is cast in Antique Bronze, Shiny Gold Color, Chrome Color, or Pewter, enhanced with baked-on enamel, and sealed under a heavy-duty clear coat engineered for long-term durability and exterior installation.

Please be specific in the notes section when describing your design. If you are uncertain about layout, wording, or symbolism, give us as much detail as possible and if there are questions, someone will reach out to you via email or phone call to clarify your desires.

Production Timeline:
Custom medallions sized 2″–6″ require approximately 6–10 weeks after final written eSignature approval. Medallions 6″ and larger require 3–6 months. Medallions 12″ and larger may require up to 10 months due to tooling, casting scale, and finishing requirements.

Your Custom Order Includes:
• Three (3) finished medallions in 2″, 3″, or 4″ sizes
• OR two (2) finished medallions in the 6″ size
Production begins only after written eSignature approval is received.

Orders for medallions larger than 6″ include one (1) finished medallion with extended production timelines.

Additional medallions may be purchased at standard inventory pricing at the time of order. Personalized designs (names, dates, badge numbers, ranks, logos, business identifiers, etc.) are typically produced only in the quantities ordered. Requests for additional quantities must be submitted prior to eSignature approval and are not guaranteed.

Upon purchase, all submitted information is forwarded to our art department for mockup creation.

Artwork Revisions:
Each paid custom order includes up to three (3) artwork revisions based on the original approved concept. Additional revisions or material concept changes will incur additional artwork fees. No additional work will proceed until such fees are approved and paid.

After final artwork is issued, you will receive an electronic approval form. Once signed, the design is locked. No artwork, quantity, or specification changes may be made after approval.

Intellectual Property & Licensing:
By submitting artwork, logos, insignias, seals, emblems, trademarks, or other protected materials, you represent and warrant that you own the rights or have obtained proper authorization to reproduce such material.

Hero Medallions does not assume liability for unauthorized use of copyrighted, trademarked, or otherwise protected materials submitted by customers. You agree to indemnify and hold harmless Hero Medallions and its affiliates from any claims arising from the use of submitted artwork.

We reserve the right to decline production of any design that infringes upon intellectual property rights, violates licensing agreements, or conflicts with legal restrictions.

Unless otherwise agreed in writing, custom artwork created by Hero Medallions remains the intellectual property of Hero Medallions for manufacturing purposes. We reserve the right to reproduce or display non-personalized design elements for portfolio or promotional use.

By completing this custom order and submitting payment, you acknowledge that you have read, understand, and agree to all Custom Order terms, including the Intellectual Property and Licensing provisions stated above.

Due to fluctuations in copper, tin, bronze, coatings, labor, and transportation costs, custom medallion pricing is subject to change without notice.
